Provisioner update and config update.

We will be completing maintenance on the provisioner system during this time. The config update will resolve the following issue -

Poly devices with MAC OUI b8:87:88 are not recognized as Polycom because the OUI is missing from ouis_by_brand.polycom in constants.json.

The update is to 5.12.12.5 which contains various bugfixes.
